Access someone else's medical records
There are 3 ways to access someone else's medical records
You may be a parent, guardian or a carer. In most cases you will need permission from the patient to access their notes or to represent them.
If you just need access to their online record to order a prescription, book an appointment or complete a questionnaire you will need to fill out the Proxy Online Access form - please click on the named link below
If you just want to talk about their medical needs, discuss test results or request a prescription you will need to fill out the Consent to Access Medical Records form - please click on the named link below.
