Proxy access process 

Please be aware that there are cut off points for each age group. These are as follows:

0-11 (no verbal consent required and no form)

11-14

14-16

16-18

18-21

21+ (ongoing)

 

To allow proxy access to your online record the Practice will require the following:

  • Please be aware that all requests can take up to 30 days to process. This may take longer if GP needs to review the request.
  • A completed and signed proxy access form (to be completed by the Patient and representative)
  • Verbal consent from the patient to give the Proxy permission (a member of the team will contact the patient)
  • A form or online application is required to be completed, alongside verbal consent at each age group.
  • You will need to resubmit another request to continue to enable proxy access after every cut off anniversary
