Health and Wellbeing Team
The BoA Health and Wellbeing Team are here to support the health and wellbeing of patients 18+ to help them make a positive and lasting change to their health with a personalised approach.
We aim to encourage patients every step of the way, giving them the tools and techniques to last a lifetime. The team is made up of Social Prescribers and Care Co-Ordinators who work together alongside the surgery to provide the best possible care tailored to patients needs.

Can support patients with issues such as:
- Social isolation and loneliness
- Emotional wellbeing
- Health lifestyle choices
- Getting out and about
- Life changing events
- Long term health conditions
- Accessing work training
- Bereavement support
Care Co-Ordinators
Tanya - Patient Facing Care Co-Ordinator
Work closely with the social prescribers supporting patients with:
- Welfare checks
- Referrals to social services
- Signposting to care
- Blood pressure
- Height
- Weight
How to Access the Service
Any member of staff at the surgery will be able to refer you to the team.
You will then be contacted by us to arrange an initial consultation. This first appointment may be by phone, face to face or as a visit.
BoA community hub
The Hub includes a foodbank, access to computers, support completing applications for jobs and benefits, debt and welfare benefits advice, money management together with general advice and support plus facilities including a meeting space.
Wiltshire Councils Health Improvement Coaches
Wiltshire Councils Health Improvement Coaches work on a one-to-one basis providing support to improve health and wellbeing.
If you would like to improve your mental wellbeing, stop smoking or lose weight, but do not know where to start, a Health Improvement Coach may be able to help. The service is open to anyone over 18 years of age.
Your Health Improvement Coach will concentrate on behaviour associated with ill health. Clients can be signposted to access other services and activities where appropriate.
